How do I submit an allegation or concern?
You may file your allegation or concern by phone, by mail, or by submission form.
Who will investigate my allegation or concern?
If a formal investigation is opened, our Management will assign a supervisor to investigate. The supervisor may contact you further to gather more information or if clarification is needed. The supervisor will provide you with their name and a phone number to contact them. You can call this person during normal weekly business hours if you have questions.
What happens after I file a concern?
The supervisor will ask you to give a written statement, if one wasn't already provided. You may be asked to provide details of the incident, your contact information, witnesses and provide access to photograph any injuries or property damage.
How long will the Investigation take and when will I find out the results?
We make every attempt to complete an investigation within thirty days of receiving notice. If we cannot, we will contact you. When the investigation is complete, we will send a letter that explains the findings and the employee will also be notified of the outcome. Our company reserves the right to discipline or not discipline those involved.
